Outpatient Information
Surgery Information
Due to scheduling protocols and hospital regulations, we are unable to
give you an exact time of surgery until the day prior to your surgery. WE
ARE UNABLE TO CHANGE YOUR TIME OF SURGERY ONCE IT IS SET. We can work with
you to determine a convenient date but again we are not able to guarantee
the scheduled time. Please check your calendar before scheduling to be sure
you can block the surgery time without any other consideration. Only if it
is a medical emergency can your surgery date be canceled or rescheduled.
Assistant Surgeon
For your benefit, your surgeon may elect to have an assistant surgeon
present at the time of your surgery. The assistant surgeon may be a physician,
physician assistant, or a nurse practitioner. The assistant may not always
be a member of the SOAR group. The usual fee for the Assistant Surgeon is
approximately 25% of the surgeon's fee. Some insurance carriers do not provide
benefits for Assistant Surgeons. Where insuance carriers deny coverage, we
will reduce the fee to $250. You will be billed separately for this service.
Anesthesia
Your anesthesiologist will not be assigned until late the day prior to
your surgery. SOAR does not have a decision in who will be your anesthesiologist.
Questions should be referred to the Associated Anesthesiologists Medical group
at 650-323-0617.
Insurance & Billing
The Surgery Scheduling Department will contact your insurance company to verify
your coverage and to obtain any authorization / pre-certification for your
surgeon's services.
If you have not given our office complete insurance information; please contact
the Surgery Scheduling department immediately.
WE STRONGLY RECOMMEND THAT YOU CONTACT YOUR INSURANCE AS WELL TO ENSURE A
BETTER UNDERSTANDING OF YOUR BENEFITS AS THEY PERTAIN TO YOUR SURGERY. AUTHORIZATION
FOR SURGERY BY YOUR INSURANCE COMPANY DOES NOT GUARANTEE PAYMENT OF BENEFITS.
Insurance Billing
Our office will submit a bill to your insurance company. If we do not
have a contract through a PPO (Preferred Provider Organization) and do not
receive payment within 30 days you will receive a billing statement which
is due in full upon receipt. If your insurance is a PPO plan and there is
a patient liability balance, you will receive a statement for any remaining
balance due in full upon receipt.
You will receive a statement an (EOB) Explanation of Benefits from your insurance
company from four seperate providers of service. Your surgeon, the facility,
the anesthesiologists and the assistant surgeon if used. If you have any questions

Please note that even though the doctor orders DME for your surgery, it is
not mandatory that you use this equipment. We are not responsible if your
insurance does not cover the cost of Durable Medical Equipment. It is your
choice to use DME and call your insurance company for benefit coverage.
Preferred Provider Organization
If your insurance requires you to see a contracted physician/surgeon
on it's list, please check to make sure ourr surgeon is a member of your plan.
This is the patient's responsibility.. Not all physicians in our office participate
with all of the same insurance plans.
Charges
The Surgery Financial counselor can give you an estimate of the surgeon's
fee, however for questions regarding any out patient facility charges you
must call 650-723-6181 for Stanford and for Waverley, 650-289-1648. Your billing
information is provided to all facility/providers involved in your case. However,
all fees incurred for services outside of SOAR, such as the assistant surgeon,
anesthesiologist, hospital, etc, is not our responsibility. We can not guarantee
all other providers of services are contracted with your insurance.
Usual, Reasonable and Customary
Our fees are set according to our geographical location, the high level
of expertise and quality of care our physicians provide. Some insurance companies
that we are not contracted with will indicate that our fees are above their
usual and customary rate. You will still be responsible for the full amount
of the charge. The only time we allow for an adjustment is if your insurance
is a PPO with which our organization has a contract.
Pre-Operative Instructions
We require that all patients comply with the following instructions:
It is imperative that you do not eat or drink anything (including water)
after midnight the night prior to you surgery.
You must arrive at Waverley Surgery Center 1 hour prior to your surgery time.
If your surgery is scheduled at Stanford or Sequoia Hospital you must arrive
2 hours prior to your scheduled surgery time.
Your Surgery Coordinator will call you with this time one day prior to your
surgery date. Please be sure the scheduler has all your contact numbers. Please
keep your cell phone on, on that day.
Wear loose fitting comfortable clothing to and from the hospital. Preferred
button down shirts, no "pull over" type shirts with shoulder surgery.
Leave all valuable items at home.
Please bring a written list of all your medications and the dosage of each
medication to your pre-op appointment with our Nurse Practitioner. This is
important for your anesthesiologist.
On the day of your surgery you will not be able to drive yourself home nor
can you use a taxi. Please make arrangements for someone to drive you.
Do not take aspirin or aspirin-containing products at least 7 days prior
to your surgical procedure. This includes all anti-inflamatory medications.
Please do not sunbathe at least one week prior to surgery due to the risk
of sunburn.
Please inform your surgery scheduler if you are diabetic, have a latex allergy,
or sleep apnea.

The Day Of Your Surgery
Wear loose, comfortable clothing so that you can change easily after surgery.
Do not wear makeup, nail polish or jewelry.
Bring your health insurance card and insurance forms with you.
Please leave all valuables at home.
Please bring any X-rays, MRI scans, lab studies, physician orders etc. that
may relate to your surgery.
When You Arrive
To ensure that there is adequate time to fill out the necessary forms and
prepare for surgery, you should arrive at your designated facility at least
1 to 1-1/2 hours before your scheduled procedure. Family members and friends
are welcome to join you but must wait for you in the receotion area.
Upon arrival at your facility, you should register at the Patient Registration
Area located inside the reception area. You will then be escorted to a preparation
room, where you'll be asked to change into a hospital gown. At this time,
we'll perform a simple physical examination ot check your weight, blood pressure
and temperature, and insert an IV. From the pre-operative area, you will then
be taken to the operating suite for your procedure.
During the time that you are in surgery, your family and friends are welcome
to wait in the reception area.
